NorthLakes Community Clinic is excited to offer a school-based preventative dental program for all children enrolled in your school. A Registered Dental Hygienist will come on-site to provide dental services at NO COST, meaning families will never receive a bill for services. This program is designed to complement your child's regular dental care and can be used alongside services provided by your family dentist. If your family doesn't have a regular dentist, we would be happy to assist you in finding one.
This program includes:
- Oral health education
- Free dental supplies offered at each visit
- Oral health assessment by a Registered Dental Hygienist
- Fluoride varnish applications (2-3 per school year)
- Dental sealants & sealant replacement when needed
Sign up online, Click this link https://sealasmile.wisconsin.gov/ConsentOnce you have completed the Online Consent, it will automatically send it to our Dental Outreach Program Coordinator. After each visit, you will receive a report describing your child's oral health and the services that were provided. If any dental concerns are suspected, a dental referral will accompany the report. A copy of the referral will be shared with your dentist(if listed). This program is funded in part by Wisconsin Seal-A-Smile in collaboration of the Children’s Health Alliance of Wisconsin and the Division of Public Oral Health Program.
